This is an Adobe file, set up as an 8-1/2" x 14" sheet. It will print on an 8-1/2 x 11 but it'll be smaller type. Be sure your printer is set to "landscape" (may do it automatically check the little print picture)

ST-19 Minnesota Sales Tax Form
